Step 1: Complete New Client Forms
Once you fill in all of the boxes a submit button will appear at the bottom of the page (you must fill out all portions of the paperwork to submit it.), make sure to submit the paperwork so we can receive and verify insurance.
If your paperwork is not received 48 hours before your session, your appointment will be canceled.
Step 2: Schedule Initial Appointment
Important: Use the same email on your new client forms when scheduling your appointment. This will avoid creating a duplicate account in the Client Portal.

- Our office staff is not located at this location but the therapist is aware of your appointment and will come out to get you at the appointment time.

Our office staff is available Monday thru Friday from 8 am to 5 pm via phone 678-833-5395 or email email@example.com for any questions, scheduling, and/or billing needs. The office staff is not available after 5 pm or on weekends. All emails and voicemails are dated and time stamped and will be answered the following business day M-F.
**Please be aware of our cancellation policy: More than 15 minutes Late, No Shows, Canceling or rescheduling within 48 hours of your appointment time or within 72 hrs of Monday or Weekend appointments will be subject to a cancellation fee.**
